If you are a parent or guardian of a child less than 8 yrs of age, please fill out the Pediatric History instead of the Health History form.

Some other things to consider before you come in for your eye examination:

  • Please come to the clinic early if you need an exam AND you are interested in looking for new glasses. You might also consider looking at glasses on another day. Once your eyes are dilated, looking at oneself up-close in a mirror becomes far more difficult, and thus this is a task easier done prior to your examination, or on a different day altogether.
  • Dilation is an important part of a complete and comprehensive exam. It is highly recommended by both of our doctors and since it leaves near vision blurry for a few hours, you might bring a driver, or arrange for a ride if you are uncomfortable driving with dilated eyes. Upon request, we can give you a reversal drop to bring the pupil size back down in approximately ninety minutes to one hour.
  • In some instances during your exam, the doctor may recognize a need for additional testing, which we’ll discuss with you as necessary. Normally this testing still fits within the one hour we schedule for complete eye examinations.
